Service Agreement for the-Labs Only Women’s Purpose Community Health Plan

This Service Agreement ("Agreement") is entered into as of the purchase of this program  between the purchaser and Jennifer Seine,, L.Ac., Certified Functional Medicine Practitioner, hereinafter referred to as ("Provider"). By purchasing the VIP Women’s Purpose Community Health Plan, Client agrees to the terms outlined below.

1. Services Provided

The VIP Women’s Purpose Community Health Plan includes:

• Comprehensive Micro-Nutrient Testing.
• Hormone and Adrenal Testing.
• Personalized Functional Medicine Action Plan.
• Health Coaching sessions with Tracy Hall. (for VIP members only)
• Complimentary evaluation of blood lab work.
• Nutrition and Movement Reset Program.

2. Responsibilities of the Provider

Provider agrees to:

• Deliver all outlined services in a professional and timely manner.
• Use best practices in functional and integrative medicine to develop Client’s plan.
• Provide guidance, education, and support through consultations and coaching sessions.

3. Responsibilities of the Client

Client agrees to:

• Provide accurate and truthful health information.
• Complete all required lab tests and forms in a timely manner.
• Follow through with recommendations and action plans to the best of their ability.
• Maintain relations and up to date testing with their primary health care provider.

4. Fees and Payment

• Payment is due in full upon registration unless a payment plan has been arranged.
• Payment plans, if offered, will be outlined in a separate agreement.

5. Refund Policy

Due to the nature of this program and the resources provided, no refunds will be issued once services have begun. Exceptions may be made at Provider’s discretion for unforeseen circumstances.

6. Disclaimers

Provider is a licensed healthcare provider and will use expertise to develop individualized recommendations. However:

• Results and outcomes vary by individual and cannot be guaranteed.
• Services are not intended to replace emergency care or primary medical treatment.

7. Confidentiality

All information shared by the Client will be kept strictly confidential and used only for the purposes of creating and managing the Client’s health plan.  Tracy Hall & Jennifer Seine will work together, and have your permission to discuss your health details in order to enhance your care.

8. Termination of Agreement

This Agreement may be terminated by either party in writing with reasonable notice. Any services provided up to the termination date will be billed accordingly, and the Client will remain responsible for outstanding fees.

9. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of California, Georgia and by the National Accreditation of Acupuncture and Oriental Medicine.

8. Hold Harmless and No Liability Clause

The Client agrees to hold harmless and release Jessica Joines and the Women's Purpose Community from any and all liability arising from participation in this program. The Women's Purpose Community and its affiliates are not responsible for any outcomes or results of the services provided in this program. Any reliance on the guidance or resources shared as part of this program is at the sole discretion of the Client.

9. Termination of Agreement

This Agreement may be terminated by either party in writing with reasonable notice. Any services provided up to the termination date will be billed accordingly, and the Client will remain responsible for outstanding fees.
